The condition is caused by a recurrent (in at least 9 families) 5'UTR hotspot single-nucleotide deletion, which causes a relative switch in transcription start sites for POMP, predicted to decrease levels of POMP protein in terminally differentiated keratinocytes. Ichthyosis is a prominent feature of the condition.
